.1. If you ALWAYS meet in the same indoor space only one version of this form is required.
2. If you USUALLY meet in the same indoor space but on occasion visit other indoor or outdoor spaces you are required to complete a form for EVERY visit in addition to one form for your usual meeting place.
3. If you ALWAYS meet outdoors, or in various indoor locations you MUST complete a form for EVERY such meeting.
